New Client Registration

Dr. Shirin participates in several insurance plans through a number of major insurance carriers. To become a counseling client, please click the link below and complete the new client registration packet in its entirety. Once the registration packet is complete, our benefits coordinator will verify your insurance benefits. Once benefits are verified, Dr. Shirin will reach out to you via email to set up an appointment. Telehealth sessions occur between hours the hours of 10am-2pm Tuesday through Thursday and 4:30pm to 8:30pm Monday to Thursday, by appointment only. If you would like to make an appointment please complete the required intake forms and insurance information as found on the link below. Please be sure to complete the insurance form in its entirety so that your insurance benefits can be verified prior to the first appointment.
